Gift certificate order form

Be sure to include your address, so that I can mail it to you. $30.00 is equal to 1 hour of cleaning per maid (there is a 2 hour minimum for 1 maid) The gift certificates are only good for homes within 25 miles of Massillon, Ohio 44647. You can pay via PayPal, uniquelymaid@yahoo.com, cash app @$uniquelymaidcleaning, or venmo @Jenni-Bruce. As soon as the form and payment are received I will mail your gift certificate.
